Grantmaking

Individual Grant Criteria and Guidelines

  1. The Foundation gives special attention to requests that:
    • help individuals with developmental disabilities transition into community living  
    • make minor home improvements that enhance accessibility and quality of life 
  2. The following criteria will be applied to grantee selection. Individuals requesting grant funds must submit the following verification documents with the grant application.
    • Documentation that individual has a developmental disability
    • Documentation that individual is currently living in a community setting or planning to live in a community setting
    • Documentation that individual has a financial need (Medicaid, SSI or Waiver funding, or personal tax return)
    • The request is not covered by governmental-entitlement-program funding
    • Estimates or invoices to validate cost of supplies or services to be covered by grant funds
  3. Grant requests may be made for up to $1,000.
  4. All information submitted in the grant application is confidential and will not be used for any other purpose.
  5. Grants funded through the Community Living Fund are objective and non-discriminatory. Grants are awarded based on proof of financial need.
  6. Individuals are eligible to receive grant funds once every five years.
  7. Funds received must be used for the approved purposes within 3 months.
  8. Applicants must comply with submission requirements and the terms of the grant agreement.
  9. All funding decisions of the Community Living Fund Committee are final.
  10. The Foundation does not consider grant proposals for personal needs (computers, medical or dental bills) or services covered by government programs or within an individual’s financial means. In addition, grants are not awarded for items purchased, built or modified prior to grant submission.
  11. To initiate the grantmaking process, an individual or guardian must complete a grant application form and return it by postal mail to the Foundation For the Challenged Community Living Fund Committee. 
  12. Applications can be submitted at any time. Grants are made four times per calendar year.

  13. Applicants will be notified by mail within 30 days of grant review month.

Applications without requested documentation will not be considered for funding.
